• Evaluation for Qualifying Exam
  To be completed and returned to the DGS* by all advisory committee members after the oral qualifying exam for each student.
• Annual Graduate Student Evaluation
  To be completed and returned to the DGS* by all advisory committee members after each annual committee meeting.
• Annual Review of Progress
  To be completed by the student and his/her advisor each year, and returned to the DGS by July 31. 
• Seminar Evaluation by Audience
  To be completed by all audience members and returned to the seminar coordinator after each graduate student seminar.
• Graduate Advisory Committee Evaluation of Seminar
  To be completed and returned to the DGS* by all advisory committee members in attendance after each graduate student seminar.
• Dissertation Defense Evaluation
  To be completed and returned to the DGS* by all advisory committee members after the thesis or dissertation defense.
